New voters and those left out of the draft SIR electoral rolls can apply for inclusion through Form 6, provided they meet the eligibility requirements and are not already enrolled in another electoral roll.Applicants need to provide their personal details, address proof and supporting documents before submitting the application online.
How to apply through form 6
Visit voters.eci.gov.in and select New Voter Registration → Fill Form 6. Enter your mobile number and register using the OTP received on your phone. Next, select your state, district and Assembly constituency.Enter your name as it appears on your Aadhaar card. The Kannada version will be generated automatically, but you can make corrections using the Kannada keyboard option. You will also need to upload a passport-size photograph with a white background and a file size of less than 2 MB.The form then asks for details of a relative, parent, spouse or, where applicable, a legal guardian. You must also provide a mobile number or email ID belonging to you or a relative. Your Aadhaar number can be entered if available.Next, provide your gender and date of birth and upload an accepted document as proof of age. You will also need to enter your present address and submit a self-attested address proof. This document may be in your name or in the name of a parent, spouse or adult child who is already enrolled as an elector at the same address.If applicable, applicants can also provide their disability category and upload the relevant certificate.
Family and previous electoral roll details
Where applicable, applicants must provide details of a family member already included in the electoral roll at their current address, including their EPIC number.The form also asks applicants to indicate whether their name or a family member’s name appeared in the electoral roll of the last SIR. You will need to select one of three options: your own name was included; your parent’s name — father, mother, grandfather or grandmother — was included; or neither your name nor your parents’ name was included.Applicants must also provide the names of their father, mother and spouse, along with their EPIC numbers if available.
Complete and submit the application
Once all the details have been filled in, complete the declaration using the OTP sent to your registered mobile number. You will then need to complete the e-sign process using your Aadhaar number and the OTP received.After submission, save the reference number and download the acknowledgement. Keep these details safely for future reference.
Documents accepted as proof of date of birth
The listed documents include a PAN card, Indian passport, driving licence, Class 10 or 12 certificate issued by a state board, CBSE or ICSE, birth certificate issued by the competent authority, and Aadhaar.
Documents accepted as address proof
For address verification, applicants can use a water, electricity or gas bill that is at least one year old, a registered sale deed for an owned house, a registered rent or lease deed, Revenue Department land records including Kisan Bahi, an Indian passport, a current passbook from a nationalised or scheduled bank or post office, or Aadhaar.
