Lucknow: Doctors at SGPGIMS successfully performed a complex hip fracture surgery on an elderly patient who had undergone placement of two drug-eluting coronary stents just two months earlier following a major cardiac event.The patient was on dual antiplatelet therapy (DAPT), making the surgery particularly challenging as doctors had to balance the risk of potentially serious bleeding against the danger of stent thrombosis if the medication was stopped.Doctors said hip fractures in elderly patients require timely surgery as prolonged immobilisation can increase the risk of complications including pneumonia, blood clots and pressure sores. However, major surgery soon after coronary stenting carries significant cardiac risks.A multidisciplinary team of orthopaedics, cardiology and anaesthesiology specialists planned the procedure after assessing the patient’s cardiac and anaesthetic risks. Ultrasound-guided continuous spinal anaesthesia was used to maintain stable haemodynamics during surgery, followed by intensive postoperative monitoring.The surgery was led by Dr Pulak Sharma, head of the department of orthopaedics, while anaesthetic management was handled by Dr Vansh Priya. Other members of the orthopaedic team included Dr Furquan, Dr Sujeet, Dr Abhishek and Dr Vikas.Three weeks after the procedure, the patient is recovering well and has started walking independently with support and resumed routine daily activities.Doctors said the case highlighted the importance of timely intervention and coordinated multidisciplinary care in managing elderly patients requiring major surgery soon after coronary stenting.
