Surgical grant for prevention of disability
Under the scheme, a maximum grant of Rs 6 lakh is given to the eligible person to cover the cost of approved surgical treatment.

The Yogi government in Uttar Pradesh has formulated many schemes for the poor, deprived and persons with limited financial resources. One of such schemes is the scheme of financial assistance for specific surgical procedures to poor and helpless persons with disabilities. The goal is to prevent, correct, or reduce disability through timely medical intervention.

Financial benefits

·Maximum grant of Rs six lakh per eligible person to cover the cost of approved surgical treatment.

Eligibility criteria

Applicants must satisfy all of the following conditions to receive the benefits of the scheme:·Annual income of the applicant (or guardian) from all sources should not exceed Rs sixty thousand.·Should be permanent resident of Uttar Pradesh or resident of the state for at least 5 years.·Necessarily be an Indian citizen.·Not found guilty in any criminal case.

Covered medical treatments

The grant covers specific surgeries across various categories of disabilities:·Visually Handicapped: Intraocular lens implant, Cranioplasty, and Cranial repair.·Hearing Impaired: Cochlear implant, Tympanic membrane repair, and Mastoid surgery.·Orthopedically Handicapped: Includes S P Nail operations, artificial prostheses, external fixation, knee/hip/ankle correction, shoulder/elbow/wrist correction, post-polio correction, ligament repair, tendon transplant, and more.·Post-Leprosy-Cured Disability: Reconstructive surgeries for the hands and feet.Points to Note·The scheme is operated by the Department of Empowerment of Persons with Disabilities, Uttar Pradesh.·Official guidelines and the application form are available on the department’s website (uphwd.gov.in) under State Funded Schemes.·For the most accurate and up-to-date details (including any revisions to income limits, covered procedures, or process),one may refer to the latest official guidelines or contact the District Disabled Welfare Officer in your district.

Application process (Offline)

1.Download the prescribed application form (available on page 18 of the official scheme guidelines).2.Attach the required documents:oRecent photographoIdentity proofoIncome certificate (from all sources)oDisability certificate issued by a competent medical authorityoEstimated expenditure details of the proposed surgery, issued by the Superintendent/In-charge of a State Hospital3.Submit the completed form and documents to the District Disabled Welfare Officer (District Disability Empowerment Officer). The officer adds a recommendation and forwards the application to the Director.4.In genuine medical emergencies, the application may be sent directly to the Director.
